Millions upon millions of dollars mistakenly included on a medical bill. That is what Part I will address, human error.
Mistakes happen, that is part of life. And sometimes when we try the hardest we make the most. The same applies to entering orders. A wrong test is ordered, or wrong date or not in proper time sequence. It could happen and it does happen.From personal experience I can tell you I have been called by lab, radiology, pharmacy and even dietary questioning about an order in the computer. Fortunately not all in the same day on the same patient. Just recently lab called about an order, a magnesium was scheduled to be drawn even though one had been done earlier in the morning. Since there was no order, I canceled the new one.Safety of the patient isn't compromised purely waiting for an order. So the service is rendered first and then an order is put in, most times the department will remind you to do so. So a stat x-ray can be completed with the promise of an order later.And when the situation has calmed is an order placed? Well certainly that is the goal to charge for services rendered, but in reality sometimes it is missed.In all hospitals I have worked at nurses are required to do chart checks, be it twelve hour or twenty four hour or both, to make sure orders are properly entered and completed. This is helpful when physicians enter orders as they do in some facilities, hopefully reducing errors.As for supplies, I can give you an example. One night I was particularly busy tending to 2 ill patients. Running from task to task to make sure each had proper care. When I finally sat down I stickers from supplies all over my uniform top.When it was time to affix the stickers to the "charge cards" I had to remember which sticker and corresponding supply I used for which patient. I am sure I did just that, but with the acuity of patients nowadays and the hectic work load stickers can be misplaced.And finally I would like to relate to you a case of human error that was very well intentioned and not discovered immediately. As I have said I have worked in a variety of care settings. And this example took place in the NICU. We had the habit of placing small squares of fleece beneath the babes. This was to prevent skin breakdown and make the isolette more comfortable.How could we have guessed that that little square would be counter productive. You see in some cases the fleece made the x-ray appear more patchy and white than it was in actuality. The cause was found quickly for the patchy x-ray by a clever physician and was fixed immediately.From this brief article I hope you can see that human error albeit unintentional can cause errors on your hospital and medical bill.
